भारतीय रेलवे में नियत कालिक मेडिकल परीक्षा (PME – Periodical Medical Examination) : संपूर्ण मार्गदर्शिका

भारतीय रेलवे में संरक्षा (Safety) सर्वोच्च प्राथमिकता है। विशेषकर संरक्षा श्रेणी (Safety Category) के कर्मचारियों का शारीरिक एवं मानसिक रूप से पूर्णतः स्वस्थ होना अत्यंत आवश्यक है। इसी उद्देश्य से रेलवे प्रशासन द्वारा समय-समय पर कर्मचारियों की नियत कालिक मेडिकल परीक्षा (Periodical Medical Examination – PME) कराई जाती है, ताकि यह सुनिश्चित किया जा सके कि कर्मचारी अपने पद के लिए चिकित्सकीय रूप से पूर्णतः सक्षम (Medically Fit) हैं।

PME के लिए कर्मचारी को निर्धारित समय पर रेलवे चिकित्सालय में उपस्थित होकर स्वास्थ्य परीक्षण कराना तथा आवश्यक चिकित्सकीय प्रमाण-पत्र प्राप्त करना अनिवार्य होता है।

1. PME का कार्यक्रम (Schedule of Periodical Medical Examination)

रेलवे बोर्ड के निर्देशों के अनुसार विभिन्न मेडिकल श्रेणियों के कर्मचारियों के लिए PME की अवधि निम्नानुसार निर्धारित है—

(क) मेडिकल श्रेणी A-1, A-2 एवं A-3

इन श्रेणियों के कर्मचारियों की नियत कालिक मेडिकल परीक्षा निम्नानुसार होगी—

  • नियुक्ति अथवा प्रारम्भिक मेडिकल परीक्षा की तिथि से 45 वर्ष की आयु तक प्रत्येक 4 वर्ष में एक बार।

  • 45 वर्ष की आयु पूर्ण होने के बाद 55 वर्ष की आयु तक प्रत्येक 2 वर्ष में एक बार।

  • 55 वर्ष की आयु के बाद सेवानिवृत्ति तक प्रत्येक वर्ष एक बार।

विशेष प्रावधान

यदि किसी कर्मचारी की अंतिम PME 45 वर्ष की आयु प्राप्त करने से पूर्व के दो वर्षों के भीतर हो चुकी है, तो उसकी अगली PME अंतिम मेडिकल परीक्षा की तिथि से दो वर्ष बाद होगी तथा इसके बाद 55 वर्ष की आयु तक प्रत्येक दो वर्ष में कराई जाएगी।

(ख) मेडिकल श्रेणी B-1 एवं B-2

इन श्रेणियों के कर्मचारियों की PME—

  • 45 वर्ष की आयु पर पहली बार।

  • इसके बाद प्रत्येक 5 वर्ष में एक बार।

(ग) मेडिकल श्रेणी C-1 एवं C-2

इन श्रेणियों के कर्मचारियों की सामान्यतः सेवा अवधि में नियत कालिक मेडिकल परीक्षा नहीं कराई जाती।

हालाँकि सक्षम प्राधिकारी द्वारा आवश्यक समझे जाने पर अथवा विशेष परिस्थितियों में इन्हें भी चिकित्सकीय परीक्षण हेतु भेजा जा सकता है।

2. विशेष मेडिकल परीक्षा (Special Medical Examination)

A-1, A-2 एवं A-3 श्रेणी के कर्मचारियों को, यदि वे रेलवे चिकित्सक के नियमित उपचार में न रहे हों, तो निम्न परिस्थितियों में विशेष मेडिकल परीक्षा हेतु भेजा जाएगा—

(1) गंभीर बीमारी अथवा शल्य चिकित्सा (Major Illness / Operation)

यदि कर्मचारी—

  • किसी गंभीर बीमारी से ग्रसित रहा हो,

  • किसी बड़ी शल्य चिकित्सा (Major Operation) से गुज़रा हो,

  • अथवा आँखों का ऑपरेशन कराया हो,

तो बीमारी की अवधि चाहे जितनी भी रही हो, उसे पुनः चिकित्सकीय परीक्षण के लिए भेजा जा सकता है।

(2) 90 दिन से अधिक अनुपस्थिति

यदि कोई कर्मचारी लगातार 90 दिनों से अधिक अवधि तक ड्यूटी से अनुपस्थित रहा हो, तो उसे पुनः मेडिकल परीक्षा हेतु भेजा जा सकता है।

(3) अन्य सुरक्षा सम्बन्धी परिस्थितियाँ

यदि सक्षम चिकित्सा अधिकारी अथवा रेलवे प्रशासन को यह प्रतीत हो कि कर्मचारी की चिकित्सकीय क्षमता का पुनः परीक्षण आवश्यक है, तो उसे विशेष मेडिकल परीक्षा हेतु भेजा जा सकता है।

3. मेडिकल परीक्षा हेतु लगाया गया समय

रेलवे नियमों के अनुसार मेडिकल परीक्षा में लगाया गया समय निम्न प्रकार से माना जाएगा—

(क) यात्रा एवं परीक्षण का समय

रेलवे चिकित्सालय जाने, वापस आने तथा मेडिकल परीक्षण एवं चिकित्सकीय निर्णय में लगा पूरा समय ड्यूटी (Duty) माना जाएगा।

(ख) चश्मा, ट्रस अथवा अन्य चिकित्सकीय उपकरण प्राप्त करने में लगा समय

यदि चिकित्सक कर्मचारी को चश्मा, ट्रस अथवा अन्य चिकित्सकीय उपकरण लगाने का निर्देश देता है और उनके बिना कर्मचारी को Fit घोषित नहीं किया जा सकता, तो—

  • चिकित्सक द्वारा निर्देश दिए जाने की तिथि से

  • आवश्यक उपकरण प्राप्त कर पुनः Fit Certificate मिलने तक का समय

कर्मचारी के अवकाश खाते (Leave Account) से समायोजित किया जाएगा।

(ग) चश्मा बनवाने हेतु विशेष सुविधा

यदि कर्मचारी को पहली बार चश्मा बनवाना हो अथवा चिकित्सकीय कारणों से नया चश्मा बनवाना आवश्यक हो, तो—

  • अधिकतम 5 दिन की अवधि ड्यूटी मानी जाएगी।

  • आवश्यकता होने पर महाप्रबंधक (General Manager) इस अवधि से अधिक समय की अनुमति भी प्रदान कर सकते हैं।

4. मेडिकल अनफिट घोषित किए जाने पर अपील

यदि किसी कर्मचारी को मेडिकल बोर्ड द्वारा Unfit (अयोग्य) घोषित किया जाता है, तो—

  • वह 7 दिनों के भीतर मुख्य चिकित्सा निदेशक/मुख्य चिकित्सा अधिकारी के समक्ष अपील प्रस्तुत कर सकता है।

यदि अपील पर—

(क) मूल निर्णय सही पाया जाता है

तो अपील लंबित रहने की अवधि कर्मचारी के अवकाश खाते में समायोजित की जाएगी।

(ख) मूल निर्णय निरस्त कर दिया जाता है

तो पूरी अवधि ड्यूटी मानी जाएगी।

5. ड्यूटी पर पुनः कार्यभार ग्रहण करने में विलम्ब

यदि कर्मचारी मेडिकल रूप से Fit घोषित हो चुका है, किन्तु—

  • संबंधित पर्यवेक्षक (Supervisor),

  • नियंत्रक अधिकारी,

  • अथवा सक्षम अधिकारी की अनुपलब्धता के कारण

वह समय पर कार्यभार ग्रहण नहीं कर पाता, तो ऐसी देरी की पूरी अवधि भी ड्यूटी मानी जाएगी।

नोट: समय-समय पर रेलवे बोर्ड द्वारा जारी नवीनतम परिपत्र/संशोधन प्रभावी होंगे। यदि किसी विषय पर नवीनतम आदेश पूर्ववर्ती निर्देशों से भिन्न हो, तो नवीनतम आदेश को ही मान्य माना जाएगा।


Periodical Medical Examination (PME) in Indian Railways – Complete Guide

Safety is the highest priority in Indian Railways. Employees working in safety categories are required to maintain a high standard of physical and mental fitness throughout their service. To ensure continued medical fitness, the Railway Administration conducts Periodical Medical Examination (PME) at prescribed intervals.

Every railway employee covered under the prescribed medical categories is required to undergo PME at the scheduled intervals, obtain a Fitness Certificate from the Railway Medical Authority, and continue to perform duties only if found medically fit.

1. Schedule of Periodical Medical Examination (PME)

According to Railway Board's instructions, the schedule of PME for different medical categories is as follows:

(A) Medical Categories A-1, A-2 and A-3

Employees belonging to Medical Categories A-1, A-2 and A-3 shall undergo Periodical Medical Examination as under:

  • Every 4 years from the date of appointment/initial medical examination until attaining the age of 45 years.

  • Every 2 years from the age of 45 years up to 55 years.

  • Annually after attaining the age of 55 years until retirement.

Special Provision

If an employee has undergone a PME within two years prior to attaining the age of 45 years, the next PME shall be conducted two years from the date of the last PME, and thereafter every two years until the age of 55 years.

(B) Medical Categories B-1 and B-2

Employees in Medical Categories B-1 and B-2 shall undergo PME:

  • At the age of 45 years.

  • Thereafter once every five years.

(C) Medical Categories C-1 and C-2

Employees in Medical Categories C-1 and C-2 are not normally required to undergo Periodical Medical Examination during service.

However, they may be referred for medical examination whenever specifically directed by the competent authority or under special circumstances.

2. Special Medical Examination

Employees belonging to Medical Categories A-1, A-2 and A-3, who are not under the continuous treatment of Railway Medical Authorities, shall be referred for a Special Medical Examination under the following circumstances:

(1) Major Illness or Surgery

Where an employee has:

  • suffered from a serious illness,

  • undergone a major surgical operation, or

  • undergone an eye operation,

he/she may be required to undergo a fresh medical examination, irrespective of the duration of illness.

(2) Absence from Duty Exceeding 90 Days

If an employee has remained absent from duty for more than 90 days continuously, he/she may be referred for a Special Medical Examination before resuming duty.

(3) Other Safety-Related Reasons

Whenever the competent medical authority considers it necessary in the interest of railway safety, an employee may be directed to undergo a Special Medical Examination.

3. Time Spent for Medical Examination

The period spent in connection with the medical examination shall be regulated as follows:

(A) Time Treated as Duty

The following shall be treated as duty:

  • Time spent travelling to and from the Railway Hospital or Health Unit.

  • Time spent undergoing medical examination.

  • Time taken by the Medical Officer in arriving at a medical decision.

(B) Time Spent in Procuring Spectacles, Truss or Other Medical Appliances

Where an employee cannot be declared medically fit unless he/she procures prescribed spectacles, a truss, or any other medical appliance, the period commencing from the date on which such appliance is prescribed until the employee is declared fit shall ordinarily be debited to the employee's leave account.

(C) Special Provision for Spectacles

Where spectacles are required for the first time or replacement spectacles are prescribed:

  • A period of up to five days shall be treated as duty.

  • The General Manager is empowered to relax this limit in deserving cases.

4. Appeal Against Medical Unfitness

If an employee is declared Medically Unfit, he/she may prefer an appeal before the Chief Medical Director/Chief Medical Officer within seven days of the medical decision.

(A) If the Original Decision is Confirmed

The period spent awaiting the appellate decision shall be debited to the employee's leave account.

(B) If the Original Decision is Reversed

The entire period shall be treated as duty.

5. Delay in Resuming Duty

If an employee has already been declared Medically Fit, but is unable to resume duty because the Supervisor or Competent Authority is not available to accept the joining report, the period of such delay shall also be treated as duty.
